3. Baseline Assessment
Baseline data is a set of data, used as a basis for comparison, measurement or calculation in relation to a specific objective.
There are a range of baseline assessments and level of detail can vary pending on the needs of data to help facilitate Implementation.
What sources of baseline data can you access that are there to help support or provide a good context to your project?
What gaps in services are identified after you have gathered this data?
NHS improvement have produced a matrix Example of a Baseline Assessment which can be used as a basis to help establish your own measures. You may not need this level of detail.
However, use this document as a basis to identify your own ELEMENT, DESCRIPTION and MEASURE for your Baseline Assessment and note these down to input into your Implementation Summary.
